Xét nghiệm Anti-Xa là một xét nghiệm máu dùng để đo hoạt động của thuốc chống đông heparin, giúp điều chỉnh liều lượng Heparin để đảm bảo an toàn và hiệu quả trong việc ngăn ngừa và điều trị cục máu đông, đặc biệt hữu ích cho những Người có các điều kiện đặc biệt như béo phì, suy thận, hoặc có các chất gây nhiễu khác.

UFH (Heparin không phân đoạn): 0,3-0,7 IU/mL
LMWH (Tiêm 2 lần/ngày-Enoxaparin, Dalteparin): 0,5-1,2 IU/mL
LMWH (Tiêm 1 lần/ngày): 1,0-2,0 IU/mL
Fondaparinux: 0,5-1,5 IU/mL
Rivaroxaban, Apixaban, Edoxaban: Giá trị thay đổi tùy thời điểm lấy mẫu
Kết quả bất thường:
Anti-Xa quá thấp (< 0,3 IU/mL đối với UFH hoặc < 0,5 IU/mL đối với LMWH): Nguy cơ huyết khối do hiệu quả chống đông kém.
Anti-Xa quá cao (> 0,7 IU/mL đối với UFH hoặc > 1,2 IU/mL đối với LMWH): Nguy cơ chảy máu do quá liều Heparin.
Giá trị thay đổi theo thời gian dùng thuốc, chức năng thận và cân nặng.

- Người đang điều trị bằng Heparin (UFH hoặc LMWH) cần theo dõi hiệu quả thuốc.
- Người có nguy cơ cao bị huyết khối hoặc tái phát huyết khối.
- Phụ nữ mang thai có tiền sử huyết khối hoặc Hội chứng kháng phospholipid(APS).
- Người suy giảm chức năng thận đang sử dụng LMWH (do thuốc thải trừ qua thận).

- Nếu đang dùng UFH: Lấy mẫu 4-6 giờ sau khi truyền Heparin. - Nếu dùng LMWH (Enoxaparin, Dalteparin): Lấy mẫu 3-5 giờ sau khi tiêm. - Nếu dùng thuốc ức chế yếu tố Xa (Rivaroxaban, Apixaban): Thời điểm lấy mẫu tùy theo chỉ định của bác sĩ.

After sampling

- Nếu có dấu hiệu chảy máu kéo dài tại vị trí lấy mẫu, cần báo ngay cho bác sĩ. - Nếu kết quả bất thường, bác sĩ có thể điều chỉnh liều thuốc chống đông hoặc chỉ định xét nghiệm bổ sung.
