Xét nghiệm APTT (Thromboplastin một phần hoạt hóa) được dùng để đánh giá chức năng đông máu nội sinh và theo dõi điều trị bằng heparin không phân đoạn.
aPTT đóng vai trò quan trọng trong việc chẩn đoán và theo dõi các rối loạn đông máu do Thiếu yếu tố đông máu, suy gan, hoặc các Bệnh tự miễn. Xét nghiệm này cũng được sử dụng để theo dõi quá trình điều trị bằng thuốc chống đông.

Giá trị APTT bình thường:
Dao động trong khoảng 25-35 giây (mỗi phòng xét nghiệm, sử dụng các loại thuốc thử hay phương pháp xét nghiệm khác nhau có thể có mức tham chiếu riêng)
- Ở trẻ nhỏ, kết quả bình thường có thể bị kéo dài hơn một chút và có thể bị rút ngắn một chút ở Người lớn tuổi.
- Ở Người đang dùng heparin, kết quả bình thường có thể cao hơn 1,5-2,5 lần so với giá trị tham chiếu. Tuy nhiên, APTT > 100 giây gây nguy cơ chảy máu cao, cần giảm liều heparin hoặc dùng protamin sulfat (antidote) để trung hòa (Cần thăm khám thêm với Bác sĩ điều trị).

Giá trị APTT bất thường:
Giá trị APTT kéo dài đơn độc (>35s):
- Suy giảm các yếu tố đông máu nội sinh (XII, XI, IX, VIII...) xuống dưới 20-40%
- Bệnh Von Willebrand (gây chảy máu bất thường)
- Thiếu hoặc rối loạn fibrinogen (Hemophilia A, B)
- Cơ thể tự tạo kháng thể chống lại yếu tố đông máu (Kháng đông Lupus-LA)
- Bệnh lý gan-thận (xơ gan, viêm gan cấp tính...)
- Sử dụng thuốc chống đông (heparin, kháng vitamin K-Warfarin)

Giá trị APTT ngắn: Hiếm gặp, thường không có ý nghĩa lâm sàng. Tuy nhiên, nếu giá trị APTT giảm thấp hơn ngưỡng điều trị mong muốn ở BN đang dùng heparin, cần tăng liều thuốc chống đông.

- Người dùng thuốc chống đông (heparin).
- Người có dấu hiệu chảy máu bất thường (dễ bầm tím, chảy máu mũi, chảy máu chân răng).
- Người nghi ngờ rối loạn đông máu hoặc có bệnh lý về gan.

- Giữ bông gạc tại vị trí lấy máu từ 5-10 phút để tránh chảy máu. - Liên hệ nhân viên y tế nếu có chảy máu kéo dài hoặc bầm tím lớn.
